Athletic Opportunities
MHSAA Changes Participation Physical Exam Requirement for School Year 2020-21
In response to COVID-19, the MHSAA made some changes to the requirements for 2020-21 athletic seasons, most notably, waiving the pre-participation physical exam for students who have previously had one done. Any student with a valid physical exam dated on or after April 15, 2019, will only need to fill out the health questionnaire that can be found at the following link:
2020-21 MHSAA Health Questionnaire - English Version
2020-21 MHSAA Health Questionnaire - Spanish Version
Any student-athlete who has not had sports physical dated on or after April 15, 2019 will need to set up an appointment with their doctor to have an exam completed before participating in athletics. Families wishing to still have a physical exam completed are also welcome to do so. For those student-athletes, please print and take one of the following forms to your appointment.
Physical Card/Medical History Form - English Version
Physical Card/Medical History Form - Spanish Version
Regardless of the form you fill out for your student-athlete, it must be completed in full and signed before turning it into the high school office. Student-athletes without the proper paperwork turned in, will not be allowed to practice or compete with any athletic teams.
