Free and Reduced Lunch Applications

Free or Reduced-priced meals are available for those who qualify based upon income guidelines set by the Federal Government.   Applications can be filled out online or by smartphone at www.lunchapp.com.  Paper copies must be returned to the school offices.  You must fill out a new application each school year.  If you do not submit a new form by September 30, 2020, your child will automatically be charged the full price for a meal beginning on October 1, 2020.

Pandemic EBT Program

Families with children who received free or reduced-cost lunches at school qualify for new program.

The Pandemic Electronic Benefit Transfer Program (P-EBT) provides temporary funding to address emergency food needs for families affected by the pandemic.

P-EBT food assistance benefits will go to Michigan families with students ages 5-18 that are eligible for free or reduced-price school meals. This includes families currently receiving Food Assistance Program benefits, as well as those not currently enrolled in the program. No application is necessary for eligible families to receive P-EBT benefits.  Click HERE for more information.

Athletic Opportunities

Fall Sports Practices begin:

MHSAA Changes Participation Physical Exam Requirement for School Year 2021-22

A sports physical completed by a doctor on or after April 15, 2022 is again the requirement by the MHSAA for the 2022-23 school year.   Unfortunately, we were unable to secure anyone to come in and perform physicals at school.   Both Mercy Health and Spectrum Health are offering sports physicals at their urgent cares and walk-in clinics.   NO ONE should be practicing without a sports physical on file in the middle school office.

Physical Card/Medical History Form - English Version

Physical Card/Medical History Form - Spanish Version

The form must be completed in full and signed before turning it into the middle school office.  Student-athletes without the proper paperwork turned in, will not be allowed to practice or compete with any athletic teams.
